As you will be aware from the notice below and from the letter sent to all registered patients, Dr Bose has made the decision to retire from the practice after over 30 years of service.
We will miss Dr Bose hugely and wish to celebrate his retirement with his patients and colleagues.
To this end we are holding an event at the surgery on Thursday 27th April 2017 at 1pm to allow both patients and colleagues to say goodbye and pass on their best wishes.

IMPORTANT INFORMATION TO PATIENTS OF
DR BOSE & DR BOYAPATI'S PRACTICE and
DR SONNATHI & DR VEERAPPAN'S SURGERY
TUNSTALL PRIMARY CARE CENTRE
On 1 April 2017, Dr Boyapti's Surgery will merge with Drs Sonnathi & Veerappan's Surgery to become a single practice at the Tunstall Primary Care Centre site in Stoke on Trent. Dr Bose will be retiring from practice at the same time.
Although the surgery name will change, the practice will continue to provide the same services that are currently provided. The new surgery will be named Tunstall Primary Care.
The branch surgery at Whitfield Valley Centre in Fegg Hayes will also close from 31 March 2017. All services from 1 April will be provided from the Tunstall Primary Care Centre only.
This merger will ensure that we continue to provide high quality GP services. It will also enable us to offer a wider range of services for our patients.
You do not need to take any action - your health records will automatically transfer over to Tunstall Primary Care on 1 April 2017.

Friends and Family Test
From December 2014 NHS England have asked that all GP surgeries in England complete the Friends and Family Test (FFT). The aims of the FFT are to:
- Gather useful feedback from people who use the practice services
- Identify areas where improvements can be made so prompt action can be taken
- Inform current and prospective patients about the experiences of those who use the practice services
